Two witnesses pinpoint possible causes of the Imperial Logistics structural collapse
Mr Martin of Martin and Associates, an independent structural engineer, testified before the commission of inquiry on the possible non compliances which led to the collapse of the structure. He highlighted that a number of columns were not parallel or square to the grid layouts. There were missing reinforcing to column 6 and its footings. Certain column slots were either modified or cut through to accommodate certain roof rafter beams. The size and number of fasteners that were used by the contractor to fasten the concrete tilt up panels were incorrect in size and number. There were various other construction workmanship and design issues that came under scrutiny.
Mr Nic Tellerico, of Talmac Engineering who was the structural engineer on site testified before the commission of inquiry that he was never called to site during the construction of the chemical store to undertake any inspections or to approve any steel fixings. He testified that he had refused to sign off the structure after he identified that the contractor had already cast the concrete panels and beams without his knowledge.
Mr AJ Smith, an independent structural engineer also testified before the commission of inquiry of the various non compliances with the manner in which the building was constructed. Mr Smith will continue with his testimony on Monday 28 January 2019.
Mr. Sandile Kubeka, a Department of Labour Specialist: Occupational Health & Safety is the Presiding Inspector of the formal inquiry. He is assisted by Mr Lennie Samuel, a Forensic Investigator from the Department of Labour’s Inspection and Enforcement Services (IES) branch.
The inquiry follows the 28 March 2018 incident that led to the death of four workers: Bhekuyise Moses Sibiya, France Mokhuthu Sekalu, Constandino Mapukula and Siyabonga Bhane. Four other employees sustained cracked skull, fractured spine, injuries to abdomen, and muscular skeletal system.
A total of 18 witnesses have been lined to testify before the Commission. Additional witnesses have been identified and will be called in to testify based on information presented at the inquiry.
The affected parties in the building collapse incident include: Imperial Logistics (trading as Freightmax), Tilt Up SA, Talmac Engineering, ECHO Precast, Bedrock Construction, Archi Studio, JDF Construction, Moedi Engineering, Benrob Construction, and ECHO Prestress.
