Procedure to follow when applying for any replacement of certificate

The Department of Education, under the leadership of Namane Dickson Masemola, will like to assure the community not to despair when they have misplaced or lost their Grade 12 certificate.

Below are procedures to follow when applying for a lost certificate:

  • Complete an application form (available at any district office of the department of Education)
  • A completed affidavit or sworn statement signed by a Commissioner of Oaths
  • Make a payment that is according to Examinations rates (the amount changes every year)
  • Submit the complete application with correct names and ID number as it appears on the ID book, attach the affidavit, certified ID copy and certified copy of statement of results
  • Keep the receipt as proof of payment until the certificate has been received.

The department has various transactions which can be applied for: Combination of results (NB: this is not a combination of marks).

In most instances, the department receives requests for changes to personal information on the certificate:

  • This may only be made within six months of the issue of the certificate and no later than a year after writing the examination
  • The information to be changed must be an administrative error made at the time of registration
  • No change made with the department of Home Affairs will be made to a certificate. The candidate will use the official notification from Home Affairs as proof that they are the rightful owner of the certificate
  • A detailed affidavit must be supplied clearly indicating the change required, as well as the reasons why these corrections were not made earlier in the process vis-à-vis first preliminary schedule of entries, upon receipt of the time table, upon receipt of the statement of results prior the issue of the certificate.

The usual turn around time for applications is up to six weeks after receipt; however this can be delayed for a variety of reasons including the following:

  • Information on the application form is not according to the information on the certificate issued, e.g. surname, names, year of examination etc.
  • Incomplete application forms
  • Technical failures
  • Human error

Clients requiring assistance regarding their own information are free to contact Department of Education as it is the responsibility of the department to ensure that any request made has been made by the correct person to avoid corrupt activities.
