Government Pensions Administration Agency CEO dismissed
The Minister of Finance, Mr. Enoch Godongwana, has dismissed the Chief Executive Officer of the Government Pensions Administration Agency (GPAA), Ms Kedibone Madiehe, with effect from 8 September 2026 following a sanction of dismissal pronounced by the Chairperson of the disciplinary hearing.
The disciplinary process emanated from the findings of several forensic investigations initiated by the National Treasury in response to allegations, whistle-blower tip-offs and media reports related to irregularities involving the GPAA’s procurement of new head office accommodation, a security services contract, and corporate uniforms for GPAA.
Ms Madiehe was placed on precautionary suspension when the allegations arose against her as the Accounting Officer of the GPAA to allow the investigative processes to be undertaken.
Following findings of serious procurement irregularities and acts of misconduct against the Chief Executive Officer through the various investigations, a formal disciplinary process was initiated in terms of the Disciplinary Code applicable to Senior Management Services in the public service and several charges were brought against Ms Madiehe.
The disciplinary hearing, chaired by a retired High Court judge and with legal representation for both the employee and employer, commenced on 2 February 2026 and culminated in a guilty finding delivered on 24 August 2026 in that Ms Madiehe had, among other things, breached the PFMA and the GPAA’s own supply chain management and delegation prescripts, breached her fiduciary and contractual duties, shown gross dereliction of duty, prejudiced the GPAA, and exposed it to financial and legal risk. A sanction of dismissal was ultimately pronounced by the Chairperson on 7 September 2026.
Minister Godongwana gave effect to the sanction pronounced by the Chairperson as required by section 16B(1), read with section 17(1)(a), of the Public Service Act, 1994 and a letter of dismissal was issued to Ms Madiehe on 8 September 2026.
Mr Job Stadi Mngomezulu, the Deputy Director-General: Corporate Services at GPAA, will continue as Acting Chief Executive Officer of the GPAA until the requisite recruitment processes to fill the post have been completed. The Acting Chief Executive Officer has been required to ensure that governance arrangements and mitigation measures be immediately considered and put in place to restore the GPAA on a sound footing and ensure good governance.
“A decision of this nature, and the sanction that accompanies it, is never taken lightly. However, the evidence from the investigation and the seriousness of the transgressions required a proportionate sanction,” said Minister Godongwana. “Trust is earned through accountability.”
